Appointment Terms & Conditions 

By doing business with Summit Health including by booking an appointment with Summit Health, I acknowledge and agree to the following terms and conditions:

  1. Accuracy of Information: I declare that all information provided by me is complete and accurate to the best of my knowledge. I understand that any false information could have detrimental consequences for my health. Inaccurate information provided may result in delays, cancellation of services, or potential harm due to incorrect medical treatment.
  2. Responsibility for Travel Documents: I understand that I am solely responsible for ensuring I possess all necessary travel documents, including visas and meeting entry requirements. Failure to provide required documentation may result in refusal of service and forfeiture of any fees paid.
  3. Consultation Fees: I am aware that consultation fees apply to all visits. I am aware that consultation fees do not include the cost of vaccinations and are for the consultation service only.  Payment for these fees is due either before or at the time of my appointment. Payment must be made via card (Credit Card or Interac). Failure to pay the consultation fees may result in cancellation of the appointment and may incur additional administrative charges.
  4. Privacy Policy: I agree with Summit Health’s website privacy policy and the handling of personal information as outlined in their policy. Any breach of confidentiality or misuse of personal information may result in legal action.
  5. Cancellation Policy: I understand and agree to abide by the cancellation policy set forth by Summit Health. Failure to provide sufficient notice of cancellation may result in a cancellation fee being charged to the provided credit card.
  6. Vaccination Administration: I acknowledge that all vaccinations are to be administered exclusively at Summit Health clinics. Prescriptions for vaccines will not be issued. Attempting to obtain vaccines elsewhere or administering vaccines obtained from unauthorized sources may result in refusal of service and may compromise the effectiveness and safety of the vaccination process.
  7. Credit Card Disputes: In the event of a dispute regarding charges made to my credit card for services rendered by Summit Health, I agree to first attempt to resolve the issue directly with Summit Health’s billing department. If a satisfactory resolution cannot be reached, I agree to refrain from initiating a chargeback or dispute with my credit card issuer without first providing Summit Health with a reasonable opportunity to address the matter. Initiating a chargeback without prior communication and resolution attempts with Summit Health may result in legal action to recover the disputed amount and associated costs.
  8. Health Assessment and Recommendation: I understand that the administration of certain vaccines may be dependent on my health condition, medical history, and specific travel destinations. Summit Health reserves the right to conduct a health assessment and may recommend or refuse certain vaccinations based on medical considerations. Refusal of recommended vaccinations may limit the effectiveness of immunization and increase health risks during travel.
  9. Travel Advisory and Risk Acknowledgment: I acknowledge that travel to certain destinations may pose health risks, including exposure to infectious diseases, environmental hazards, and inadequate medical facilities. Summit Health may provide travel advisories and recommendations based on current health alerts and advisories issued by relevant authorities. I accept full responsibility for any health risks associated with my travel itinerary and understand that Summit Health is not liable for any health-related incidents or emergencies occurring during my travel.
  10. Insurance Coverage and Liability: I understand that Summit Health does not guarantee the coverage of vaccination or fees by insurance companies. Insurance coverage is dependent on the patient’s individual coverage and is not the responsibility of Summit Health. Summit Health does not provide insurance coverage for travel-related incidents and is not liable for any expenses incurred due to medical treatment, hospitalization, or repatriation resulting from illness, injury, or other emergencies encountered during travel.
  11. Appointment Availability and Scheduling: I understand that appointment availability may be limited, particularly during peak travel seasons and in response to public health emergencies. Summit Health makes every effort to accommodate appointment requests; however, scheduling is subject to availability and may be affected by factors beyond Summit Health’s control, such as vaccine supply shortages or unforeseen staffing issues.
  12. Minors and Guardianship: For minors receiving vaccination services, I acknowledge that a parent or legal guardian must accompany the minor and provide consent for vaccination. Summit Health reserves the right to refuse vaccination services to minors in the absence of parental consent or in cases where vaccination is deemed medically inappropriate.
  13. Consent for SMS Communication: By providing my phone number, I consent to receive SMS messages from Summit Health regarding my appointments, health information, and booster reminders. I understand that consent is required before receiving such messages, and I confirm that I have provided express, written, and logged consent. I acknowledge that Summit Health may be required to provide proof of this consent to wireless carriers or messaging aggregators. I can withdraw my consent at any time by contacting Summit Health or replying “STOP” to any message.
  14. Modification of Terms: Summit Health reserves the right to modify these terms and conditions at any time without prior notice. Updated terms and conditions will be posted on Summit Health’s website and will be effective immediately upon posting. It is the responsibility of the client to review and familiarize themselves with the latest terms and conditions prior to booking an appointment or receiving services from Summit Health.

I confirm that I have read, understood, and agree to abide by these terms and conditions set forth by Summit Health. Failure to adhere to these terms and conditions may result in refusal of service, cancellation of appointments, forfeiture of fees paid, and potential legal action.
